I live in the US. I have an underweight 10 year old. He eats well, but just doesnt put on weight or height.
Any ideas??

The shortest guy inthe class is always the one who gets more affection and attention....you never know he might grow to be taller than most of his classmates
Proper posture and stretching exercises can help. Straightening the back while sleeping allows decompression on the spine which makes the spinal column longer and stronger, which in turn causes stretching of the cartilage discs between each vertebra, helping to increase one's height.
Thats what his doctor also says and that is how we were approaching it, but now he is the shortest in his class and under some peer pressure so was getting concerned.
My suggestion would be to stop bothering about this.Just ensure that he is fit and active. Heredity also plays a major role in child's weight and growth. Children have high metabolism rate and it seems your kid is just doing fine.He should enjoy the fit life more than anything else. As a parent you should ensure that his diet is balanced and has all ingredients required for a growing child.
